Insurance We Do Not Accept
We do not accept any other insurance. Please note that, although we do not accept all other insurance, we do as a courtesy provide a superbill upon request only, it is a detailed receipt that includes all the information needed for you to obtain reimbursement from your insurance company. This information includes DeSalvo Chiropractic’s TIN, the providers NPI, your CPT codes, and the associated ICD-10 codes.
